Who should use this form?
This form is for individuals or organizations outside the Matanuska-Susitna Borough to submit general liability claims involving the Borough.
Can I use this form to file a complaint or report a general issue?

Does submitting a claim mean the Borough accepts responsibility?
No. Liability must be proven by the individual or organization filing the claim by submitting evidence which supports their claim.

Yes. Risk Management may follow up if more information is needed.

Yes. You may attach relevant files directly through the form.
What is the Borough’s legal position on submitted claims?
